Medication therapy management program
Our Medication Therapy Management (MTM) Program is designed to help members who have chronic diseases, multiple medications and high drug costs. The goals of the program are to improve care, enhance communication between patients and providers, enable collaboration among providers, and optimize medication use for improved patient outcomes. Members and/or their caregiver will receive their MTM services from our clinical pharmacist on staff as well as from our nurse case managers.
