Nurse case management

Case management is a collaborative process of assessment, planning, implementing, monitoring and evaluating the needs and services that members may require to ensure quality and cost-effective outcomes. Samaritan Health Plans uses an interdisciplinary clinical team approach within its case management program.

Chronic Care Support Program

This program works in coordination with the case management program to assist members who have chronic diseases or conditions to effectively manage their diseases or conditions so they can lead healthy active lives. The goals of the Program are to:

• Assist members in understanding their chronic diseases/conditions
• Decrease healthcare costs
• Overcome barriers enabling compliance with provider treatment plans
• Implement education for self-management of their disease or condition
• Provide resources to the members to manage their chronic diseases/conditions
