Choosing a Medical Home provider
To make certain that you receive the best medical care and treatment, we encourage you to choose a primary care physician to be designated as your Medical Home Provider.
Most of our members will choose a provider with a specialty type of Family Practice, Pediatrics, Internal Medicine, Obstetrics and Gynecology or a General Practitioner. This provider will know and care about you—your health, habits, preferences, history and family.
Choosing a Medical Home provider from our preferred physician list will keep your expenses lower. Office Visits to a preferred Medical Home Provider will have a simple $10 co-pay.
